Psoas compartment block

چکیده

Psoas compartment block (PCB) is a peripheral regional anaesthetic technique that blocks the main components of the lumbar plexus, namely the femoral, lateral femoral cutaneous (LFC), and obturator nerves as they run within the psoas major muscle. The psoas compartment block is also known as the posterior lumbar plexus block. A posterior approach to the lumbar plexus was first described by Winnie and colleagues. They described an approach for ‘lumbosacral’ block but provided no data on the extent of neural block. The term ‘psoas compartment block’ was coined by Chayen and colleagues to describe a loss of resistance technique with injection of solution into the ‘compartment’ between the quadratus lumborum and psoas major muscles. There have been a number of other approaches described since, including an approach by Capdevila and colleagues based on modifications to Winnie’s landmarks using computed tomography.

Early pharmacokinetic of ropivacaine without epinephrine after injection into the psoas compartment

Editor’s key points † This study investigated the early pharmacokinetics of ropivacaine without additional vasoconstrictor given as a psoas compartment block (PCB). † Ropivacaine plasma concentrations were detected within 30 s after injections. † Maximum measured plasma concentrations were measured within the first 10 min, but stayed below toxic threshold. † As some fraction of ropivacaine is s...

Psoas Compartment Blockade in a Laterally Herniated Disc Compressing the Psoas Muscle -A Case Report-

A psoas compartment block has been used to provide anesthesia for orthopedic surgical procedures and analgesia for post-operative pain. Currently, this block is advocated for relieving pain in the lower extremity and pelvic area resulting from various origins. We report a case of a 69-year-old male patient who had gait abnormality with posterior pelvic and hip pain, which were both aggravated b...

Ultrasound-guided psoas compartment block and general anesthesia for arthroscopic knee surgery: a case report.

Anesthetizing the lumbar plexus at its origin facilitates a more "complete" psoas compartment block compared to peripheral approaches. It is usually performed using surface anatomical landmarks, and the site for local anesthetic injection is confirmed by observing quadriceps muscle contraction to peripheral nerve stimulation. Ultrasound may provide guidance alone or together with the aid of ner...

The efficacy of the psoas compartment block versus the intrathecal combination of morphine, fentanyl and bupivacaine for postoperative analgesia after primary hip arthroplasty: a randomized single-blinded study.

PURPOSE Intrathecal morphine and psoas compartment block represent two accepted techniques to provide postoperative analgesia after hip arthroplasty. We designed a prospective, randomized, single-blinded study to compare these two techniques. METHODS Forty patients scheduled for primary hip arthroplasty under general anesthesia were randomized to receive either an intrathecal administration o...

The Psoas Compartment Block for Hip Surgery: The Past, Present, and Future

A posterior lumbar plexus block or psoas compartment block (PCB) is an effective locoregional anesthetic technique for analgesia and anesthesia of the entire lower extremity including the hip. Since the first description in the early seventies, this technique has been modified based on advanced knowledge of the anatomical localization of the lumbar plexus and the improvement of technical equipm...
